SUMMARY : Follow up with customers to provide a wide variety of Workforce and community resources as needed. Assist program staff by confirming employment and/or education requirements have been met to meet established performance goals.

DUTIES AND RESPONSIBILITES:

  • Adhere to the mission of SERCO.
  • Provide holistic and comprehensive case management services to a;; Follow-up participants including
    • Goal setting
    • Weekly outreach
    • Counseling services
    • Mentoring
    • Advocacy and
    • referrals
  • Write monthly case notes of follow-up participants; including all services provided within that month
  • Make sure each follow-up participant is enrolled in one of the following milestones:
    • Post-Secondary Education
    • Unsubsidized Employment
    • Military
    • Qualified Apprenticeship
    • Advanced Training
  • Collect milestone documentation every three months
  • Input milestone notes and information into TWIST system
  • Meet with individuals on an individual basis throughout the contract term regularly to discuss goals and progress
  • Maintain follow-up participant files of your caseload up to date
  • Monitor participants during internships, and collect weekly timesheets to enter participants work hours in the TWIST database for the summer and throughout the school year.
  • Monitor High School Senior participants’ academic progress to ensure they obtaining HS credential by appropriate time
  • Foster a strong working relationship with school guidance counselors, teachers, and other school stakeholders.
  • Attend all scheduled staff meetings and/or trainings
  • Engage participants in after-school activities and conduct outreach for inactive participants
  • Meer deadlines established by the Team Lead
  • Meet with program director regularly to discuss updates and progress of caseload
  • Supervise youth during drop in hours, college visits, and school trips, etc.
  • Other duties as assigned.

EDUCATION AND EXPERIENCE:

1. A four-year college degree with major emphasis in social sciences, education, or a field related in employment opportunities and public relations.

2. Six years’ related work experience will substitute for a degree.

3. Three years’ job training experience or similar activities involving individual counseling and case management.

4. Familiar and sensitive to the unique needs of economically disadvantaged individuals and families of diverse ethnic backgrounds.

5. Must have strong, effective oral and written communication skills, as well as strong interpersonal skills.

6. Bilingual (English/Spanish) preferred.

SPECIAL REQUIREMENTS:

1. Must possess a valid Texas Driver’s License and provide automobile liability insurance as required by the State of Texas.

2. Must have access to reliable transportation in order to travel within service area.

1. PHYSICAL DEMANDS : While performing duties of the job, employee is occasionally required to stand; walk; sit; for long periods of time. Employee must be able to drive for extended periods of time. Employees must occasionally lift and /or move up to 25 pounds, must be able to set up display area.

2. WORK ENVIRONMENT : Fast paced, demanding physically and mentally, will be in constant communications.
